Family Care will be offering free sports physicals to Scott High School students on Friday, May 14 in the mobile unit outside the school.

Students need to complete two forms that are in the main office — the WVSSAC physical form and a COVID screening form. Both forms need to be completed with a parent signature and turned into the office. Family Care staff will pick up the forms and schedule appointments for the students. If those forms are not completed, students cannot be seen. The forms are also attached to a post on Schoology.
